Sign up by the date printed on each
Invitational flyer.
2. Fill in the
bottom of the form completely.
a. Do not list entries for
relays.
b. HOWEVER, if you want to swim in
ANY relays, please check YES on the entry form. We will create relay
entries automatically based upon the number of swimmers that want to
swim in relays.
c. If you select YES for the relays,
then you must pay for that relay spot if we enter and pay for that
relay. PAYMENT IS REQUIRED EVEN IF THE RELAY IS CANCELLED FOR ANY
REASON..
3. After the
entries are sent in the following information
will be posted:
a. A copy of all entries
b. A list of relay swimmers
c. Amount due [cost per event, and relay cost]

4. Payment
must be made to Molly Crooks with
the entry.
a. Make check payable to Mon Valley West
YMCA
b. List number of events and invitational
name on memo line.
5.
Remember: PAYMENTS ARE NON-REFUNDABLE.
Swimmers are responsible
for all fees, except if cancellation occurs PRIOR to registration
deadline.
6. Tear off
the bottom portion of the entry form and submit it with payment. Keep
the top portion of the entry form for meet details.
